A composite memorial comprising a board, an altar and a picture.
The wall mounted oaken board is the first item to be seen as it is on the west wall of the nave, just inside the door. It has a roundel at the head containing a depiction in relief of the eagle, submachine gun and anchor badge of the Combined Operations unit and the incised inscription and names which are in upright capital roman lettering. The board is contained within an oaken frame which has a broad border of sprays of oak leaves and acorns, with a small shield at the top centre bearing a St George's cross and a similar shield at the bottom centre bearing a saltire.
The altar and picture are in the Lady Chapel, also known as the Chapel of the Guild of St Mary. The altar is by the east wall and the picture is on the wall behind and above it. The picture is a stone tablet painted with a depiction of Mary, the baby Jesus and two children, and is know as 'Unto Babes'.

12th July, 1967: The church was listed Grade I, Heritage List for England number 1215255.
2nd September, 1981: The church lies within the Tideswell Conservation Area first designated on that date.
29th January 2014: The memorial was surveyed by Roy Branson and Alan Willmot. It was in fair condition although the lower joints of the board's frame had begun to open.
