An upright oblong brass plaque on an oblong oaken backboard. The incised inscription is in upright capital roman lettering infilled in white.

c2008: The plaque was made but never erected.

2nd August 2014: The memorial was displayed in the parish hall where it was surveyed by Eric Naden. It was in good condition.
