The plaque is in the garden of remembrance on the perimeter of the site, to the east of the actual stadium.

Description:

An oblong resin plaque on a metal post. The mottled grey plaque has a bevelled edge and bears an inscription in upright sentence case roman and block lettering and a poppy emblem of the Royal British Legion.

History and Conservation
2nd August, 2014: The memorial was unveiled at a ceremony attended by Mr William Tucker, Lord Lieutenant of Derbyshire; Mr David Coleman, High Sheriff of Derbyshire; Cllr Alexis Diouf, Mayor of Chesterfield and Mr Albert Collins, former Chesterfield player.

23rd January, 2020: The memorial was surveyed by Roy Branson and Alan Willmot. It was in good condition and cared for.
