Although apparently un-named, the lane between Little Longstone and Monsall Head is sometimes referred to as Butts Road or Main Road. The chapel is on the north side of the lane. The memorial is inside the porch, on the left when entering.

Overall condition: Good. The inscription is legible.

Description:

An oblong limestone tablet with a stepped raised border. The inscription is incised in mixed styles of upright capital lettering infilled in black.

17th April, 1951: The chapel lies within the Peak District National Park designated on that date.

The chapel may lie within the Little Longstone Conservation Area.

22nd March, 1985: The chapel was listed Grade II, Heritage List for England reference 1311405.

28th April, 2015: The memorial was surveyed by Roy Branson and Alan Willmot. It was in good condition and well cared for.
